1. Applicant Information
FMCSA Lookup can prefill public registration information, including company official, power units, and driver count. The reported power-unit and driver counts will create the same number of blank rows below; add more rows if needed. Review all returned information before submitting.
2. Owner Information
If the owner is also a driver, select Yes . The owner will be added automatically to the Driver Information list. FMCSA company-official information may be prefilled after DOT lookup; verify the owner name with the applicant. Complete the driver's Date of Hire in the driver list.
